Du Quoin District 300 Kindergarten Registration Information
</element><element id="paragraph-1" type="body"><![CDATA[ Du Quoin Community Unit District 300 is looking for another large kindergarten class entering the district in Fall 2009.
By Superintendent Dr. Gary Kelly's count, that class could start out with 128--and probably more--when classrooms open in mid-August. That follows a kindergarten class that currently has 147 students in it.
Dr. Kelly said as this year progressed he felt that one of the six kindergarten teachers might follow the large class through the lower grades. However, current enrollment projections may require that all six teachers stay put.
District 300 today announced that kindergarten screening and registration will be held on Thursday, March 19th from 1:30-6 p.m., at the Du Quoin Elementary School, 845 E. Jackson Street. Children must be five years old, on or before September 1, 2009, to attend kindergarten in the fall of 2009.
Parents MUST call the Du Quoin Elementary Office at 542-2646, ext. 1051 to make an appointment for kindergarten pre-registration and screening. Parents who wish to have their registration forms completed when they report for the screening on March 19th, may pick up these forms in the Du Quoin Elementary Office at 845 E. Jackson Street.
Kindergarten students entering kindergarten for the school year 2009-2010 will be offered a full day kindergarten class, or half-day kindergarten class.
For registration, each child must have:
Original birth certificate issued by the County Clerk's Office from the county the child was born (hospital certificate will not be accepted)
Updated immunization record of your son/daughter
History of childhood diseases
Public Aid medical card (if applicable)
$20.00 supply fee, $35.00 book rental, $2.00 -- PTO money.
A date in August will be set to return physical forms to the nurse.
Illinois law requires that children entering kindergarten are up to date on immunizations. The Perry County Health Department provides this service at a very reasonable cost. CHILDREN MAY NOT START KINDERGARTEN IN THE FALL UNLESS THEY HAVE THEIR SCHOOL PHYSICAL, IMMUNIZATIONS AND REGISTERED BIRTH CERTIFICATE ON FILE.
