Registration info for Du Quoin school announced

DU QUOIN - The annual "Back To School" letter is being mailed to each student with registration information, our detailed school calendar, student accident insurance coverage information, and information regarding our free/reduced meal program.

All other information that we share at the beginning of each year will be made available at each school office, or you may access it on the district website.

New students to the district must bring a certified birth certificate and proof of residency. Physical examinations are required for students entering kindergarten, first, sixth, and ninth grades before they can enter school. If first-grade students had a physical for kindergarten, another will not be necessary. Kindergarten students entering school for the first time are required to have an eye exam by Oct. 15.

Nursing updates to immunizations: Starting the 2015-2016 school year, all students entering the sixth and 12th grades were required to show proof of meningococcal conjugate vaccination. Students entering the sixth grade must show proof of having received one dose on or after 11 years of age. All 12th graders will need to show proof of receiving two doses of MVV4, unless the first dose was administered after the 16th birthday. Hepatitis B: Students who are entering grades 6-12 are required to show proof of having received 3 doses of hepatitis B vaccine at the appropriate intervals.

• New Physical: The Child Health Examination Form revised 11/15 must be used by students who need to provide proof of the physical exam for the school year 2018-2019. The district will not accept Illinois physical exams completed on forms dated prior to 11/15.

• Fees: Book rental fees are collected on an annual basis. Book rental fees for pupils in grades K-8 will be $35 per year.

K-4 students have a class party fee of $2. Parents are asked to pay all fees as they register, if possible, unless paid at preregistration. High school students will need two separate checks: Parking sticker fee of $10 /Book Rental $55 / Yondr Fee (if applicable $15)/locker fee $5 and Drivers Ed $50.

• The middle school and the high school have adopted a new cellphone policy for students. The cellphone contract needs to be completed that will allow students to have their cellphone during the regular school day. You may request the form from your child's school office.

• Student Insurance: School time insurance is free to all students. This insurance is secondary to your primary insurance. Information on student insurance is being mailed with the registration information. As soon as an accident occurs it is the parent's responsibility to contact the school office for a claim kit. All instructions are included with the claim form. Please retain your packet of information regarding coverage. Be sure to cut out the insurance card in your insurance brochure and keep in a safe place.

• FIRST WEEK OF SCHOOL: First day of pupil attendance is Wednesday, Aug. 15.

All schools will dismiss one hour early Wednesday, Aug. 15 through Friday, Aug. 17.

• REGISTRATION SCHEDULES

• Elementary (Grades K-4): Kindergarten students who need to turn in physicals, students who did not complete preregistration, new students to the district, and students who need to pay fees should register on Friday, Aug. 3 from 8 a.m. to 3 p.m. in the Elementary Media Center. Students who completed preregistration do not need to attend. Students will be notified by mail of their homeroom teacher for the 2018-2019 school year.

• Bear Hunt for kindergarten will be from 3:30-5 p.m. Tuesday, Aug. 14.

• Back-to-School Night for Du Quoin Elementary School (grades 1-4) will be from 6-7:30 p.m. Tuesday, Aug. 14. Please take this opportunity to meet your child's teacher, see the location of their room, check out the school, and bring in your child's supplies.

• Middle school (Grades 5-8): Students attending the Du Quoin Middle School who preregistered (grades 5-8) do not need to register. Students who did not preregister and also new students should report to school from 8 a.m. to 3 p.m. on Friday, Aug. 3. The school nurse will be available to review immunization records and physical information. Sixth grade students must present a physical (a sports physical cannot be accepted). Class lists will be posted Friday, Aug. 10.

• New students from out-of-state must present a physical and immunization records, verification of grade placement, an official copy of their birth certificate, and proof of residency. Students cannot complete the registration process until these requirements are met.

• Back-to-School Night for Du Quoin Middle School (grades 5-8) will be held from 6-7:30 p.m. Monday, Aug. 13. Please take this opportunity to meet your child's teacher, see the location of their room and check out the school.

• High school registration times (Aug. 1-2):

Wednesday, Aug. 1 Freshmen (9-11 a.m. and 12-2 p.m.)

Thursday, Aug. 2 Students who did not preregister (9-11 a.m.

New Students: 12-2 p.m.

Freshman and New Student Orientation:

Thursday, Aug. 2 at 5 p.m. at Du Quoin High School.

• Back-to-School Night for Du Quoin High School (grades 9-12) will be held from 5:30-7 p.m. Friday, Aug, 17. Please take this opportunity to meet your child's teachers.
