Complete Du Quoin Back to School Registration Information

</element><element id="paragraph-1" type="body"><![CDATA[ New students to the district must bring a certified birth certificate. Physical examinations are required for students entering kindergarten, first, sixth and ninth grades before they can enter school.

If first grade students had a physical for kindergarten another will not be necessary. Kindergarten students entering school for the first time are required to have an eye exam by October 15, 2009.

Book rental fees are collected on an annual basis. Book rental fees for pupils in grades K through eight will be $35 per year. High School textbook fees are $55 plus $3 locker fee - ($58), driver's education fee - $50, parking sticker fee $5.00, yearbook $40 w/o name, $45 w/name. Cost of a yearbook will be $45 if ordered after March 15. Book rental fees, if not previously paid, will be due the beginning of school.

School time insurance is free to all students. Your child's student insurance packet will be mailed with the district handbook. Your child is covered against medical expenses for accidents that occur while attending regular school sessions. This coverage includes travel time for your child traveling directly to and from your residence to attend regular school sessions, up to one hour before or after regular classes. If your child travels to school on a district bus, the coverage extends to the travel time of the bus.

Students may purchase insurance for 24-hour coverage at $60.00 per year for grades K-12. Your child's 24 hour coverage begins the day your payment is received by the ISDA (but not before the first day of the school year), and continues until the end of the academic classes. Grades 9-12 Tackle Football Coverage may be purchased for $110.00. Football insurance coverage has been designed to specifically help to increase your protection against medical expenses related to accidental injuries sustained while playing interscholastic tackle football. Travel directly and uninterruptedly to and from practice and competition is covered as well, provided such travel is sponsored by and solely supervised by school employees. This valuable protection begins the later of the first day of sanctioned practice, or the day the payment is received by the ISDA, and extends to the last day of the season.

Parents are asked to pay all fees as they register, if possible, unless paid at pre-registration. High School students will need (3) separate checks: 1) Parking Sticker Fee - $5 2) Yearbook Fee - $40 w/o name / $43 w/name

3) Book Rental - $58/drivers ed - $50

First Week of School

First day of pupil attendance is Monday, August 17, 2009.

All schools will dismiss (1) hour early the week of August 17 through August 21, 2009.

Registration Schedules

Elementary: Kindergarten students, students who did not complete pre-registration and new students to the district should register on Thursday, August 6th from 8:00 A.M. - 3:00 P.M. in the Elementary Media Center. Students who completed pre-registration do not need to attend.

Middle School: Students attending the Du Quoin Middle School who pre-registered (grades 6-8) do not need to register. Students who did not pre-register and also new students should report to school on Thursday, August 6th from 8:00 a.m. to 3:00 p.m. The school nurse will be available to review immunization records and physical information. Sixth grade students must present a physical (a sports physical cannot be accepted).

Students who are new to the district from another school in Illinois must present a Student in Good Standing form from their previous school, an official copy of their birth certificate, and proof of residency. New students from out-of-state must present a physical and immunization records, verification of grade placement, an official copy of their birth certificate, and proof of residency. Students can not complete the registration process until these requirements are met.

Freshman & New Student Open House/Building Tour:

Wednesday, August 5, 2009, at 4:00 p.m. - 6:30 p.m. at Du Quoin High School.

Du Quoin Elementary and Middle School Items

K through 4th grade will attend Du Quoin Elementary School and 5th through 8th grade will attend Du Quoin Middle School.

CAR RIDERS:

In order to assist the flow of traffic in and around the Elementary/Middle School, we are asking those who drive children to school to follow a few simple steps.

Please be aware that Jackson and Line streets are designated as school zones for traffic purposes. Speed limits are posted. The lane directly in front of the school is one way (west to east). The west entrance into the school is one way into the lane in front of the school. The middle and east entrances are two way (in and out of school area). Busses will be entering through the west entrance and leaving through the east entrance each morning and afternoon. Those who drop students off in the morning do not use the front entrance until after 7:40 A.M. each morning. This will give all busses an opportunity to drop off students and leave prior to this time. If you need to drop off students by car before 7:40 A.M. please use the far east entrance, which follows to the rear of the middle school. Pre-K - 8 bus students will be picked up at the end of the day in front of the Elementary/ Middle School and all car riders in grades K - 8 may be picked up in the rear area of the Elementary/Middle School.

Dismissal After School:

Kindergarten students who are car riders will be dismissed at 3:00 p.m.

Students in grades 3-4 car riders dismiss at 3:04

Students in grades 1-2 who are car riders will be dismissed at 3:04 p.m.

Middle School students (5-8) who are car riders will be dismissed at 3:00 p.m.

Walkers:

For those students who are walking to school we ask that you use the sidewalks along Jackson and Line Streets. At the corner of Line and Jackson there is a crossing guard to assist students as they cross the intersection from the north. As well, Jackson and Line Streets near the school will be designated as school zones for traffic and pedestrian purposes.

Bus Riders:

For those students who ride busses, prior routes that were established will continue to operate with possible modifications. Pick up and drop off points throughout the community have been designated. If bus eligible, school personnel and/or the transportation supervisor will contact parents/guardians pertaining to this issue.

Lunch and breakfast will be served the first full day of attendance - Monday, August 17, 2009. If you need to contact the school regarding the free/reduced lunch program you may call 542-3856 or if long distance you may call collect. Parents may pay student lunches in advance (weekly, bi-weekly, monthly). Charges are not to exceed $5.00. Please refer to the handbook.

If you pay lunch money on registration day please send in an envelope with the child's name, grade, and amount of money inside envelope. Parents will be responsible for lunches purchased up until the date you complete and return your application or eligibility certification for the free/reduced lunch program.

Food allergies can be life threatening. The risk of accidental exposure to foods can be reduced in the school system if schools work with students, parents and physicians to minimize risks and provide a safe educational environment for food-allergic students. District #300 has worked to make each school building a "peanut product free" environment. We ask that each home respect this effort. If your child has food allergies or other health concerns please fill out the health concerns form in the district handbook you should receive the latter part of July and return to the school nursing office.
